Prospective national members should send their application and $11/year national dues to the FM Treasurer, Carol Smith, 1731 Daphne Street, Broomfield, CO 80020-1157.

Prospective members of a regional chapter should contact or send the form (without dues) to the appropriate chapter officer. Regional Chapters include Colorado, Midwest, Mississippi Valley, Pacific Northwest, Pennsylvania, and Southern California.

National dues are past due as of April 1. Former members of FM are strongly encouraged to rejoin.
